You do not have to follow this word-for-word. You can follow this word-for-word if you want. xD But as long as you have all of this information in your application, you can edit it or make it pretty or do whatever the heck you want to it. Feel free to add on categories if you want. You can do it in first or third person, I really couldn't care less. c:

Your application must be accepted by an admin before you begin roleplaying. This goes for staff members, too. You cannot accept your own application. Obviously. Maximum of three characters per person; a character's own pokemon do not count as a character, unless they get captured and altered into an Ex-P. Then they do. (:
